2026 Information & Requirements
Online Application Process:
To apply for SpayJax online, please:
- Access the 2026 Spay Jax Certificate PDF by clicking here.
- Make a copy of the PDF and fill it out with your information.
- Complete this form and upload the PDF alongside the other information requested.
SpayJax provides free spay/neuter surgeries and free rabies vaccines for up to six pets (dogs and cats) per qualifying household per month. Please note that an appointment is required!

To qualify, you must either:
- Receive one of the following forms of public assistance:

Instructions:
1. Call EveryPet at 904-425-0005 to schedule your pet’s surgery appointment.
2. Request a SpayJax appointment for surgery for your pet.
3. Take your pet’s city license tag to the appointment. If your pet does not have a current city license tag, you must purchase one at a cost of $22 for each pet. Individuals over 62 or disabled can receive up to three free licenses per year.
4. You must bring picture ID, proof of residence in Duval County, and proof of your participation in one of the above programs or proof of income (ex. front page of tax return, W-2, pay stubs for one month) to surgery appointment.
